First Visit

Please allow approximately 60 minutes for your first visit.

We have provided two forms for you to complete prior to your first visit: the Patient Information form and the Medical History form.

If you'd like to print out the forms and fill them out by hand, please download the following two files. You may fax the completed forms to us at 856-429-2175, or you may bring the completed forms in with you to your visit.

Forms to fax or bring with you:

On your first visit to our office, you will be greeted by one of our friendly front desk receptionists. They will ask you to check in on our patient computer. If you have not sent us your New Patient and Medical History forms prior to your visit, you will be asked to fill out the forms that day, so please arrive 10 minutes early.

Next, you will be escorted to our awesome Digital Imaging Station by a New Patient Coordinator. She will take a series of pictures of your teeth utilizing our Digital Imaging System. You'll be able to see them that day! These pictures are extremely helpful in discussing the conditions in your mouth. Sometimes Dr. Elfman may need to take a panoramic X-ray to evaluate the health, position and eruption of your teeth.

Dr. Elfman will examine your teeth and discuss your orthodontic needs. He will determine if treatment is indicated at this time. If so, he will explain the treatment. He will be happy to answer any questions or concerns you may have and discuss with you goals or expectations for you or your child. This is a complimentary visit to allow you to become familiar with orthodontics and our office. At this visit, one of three recommendations will be given:

  • Treatment is needed now
  • Treatment is not needed at all
  • Treatment will be needed, but the timing is such that it should be delayed until the appropriate growth, tooth eruption or other factors are most ideal.

Your personalized treatment plan will be presented to you. This printout will include the digital pictures of your teeth and the cost of your treatment. Our Financial Coordinator will review the treatment costs, payment plans, coordination of insurance (if applicable) and answer any questions you may have.

Concurrently, the diagnostic records can be obtained. We can usually do these the day of your New Patient visit if our schedule allows, or you may schedule for another time. The diagnostic records consist of two X-rays (Panorex and Cephalometric) and impressions for study models.

Shortly thereafter, additional appointments will be scheduled to begin the treatment as quickly as possible. We are now on the road to healthier teeth and straighter smiles!
